Hello there! Please allow up to 45 minutes for this appointment. One of our team members will call you to answer any questions you may have and to book you a Report of Findings appointment.
To book your appointment please choose the date and time. An advance payment is requested to confirm your appointment. We will send you an email with links to our various forms. Please choose the one that is most suitable to you (e.g. If you are pregnant, please use the "Pregnancy Wellness Intake" link).
